kalmasının, CD4 hücre kaybını artırdığını ve hastalığın seyrini kötüleştirdiğini tespit etmiş;
prostaglandinler gibi başka birtakım kimyasalların, kadınlar için psikolojik faydalar sağladığı;
ancak söz konusu maddelerin sindirim kanalı yoluyla (örneğin, anal ilişki) aynı etkiyi
oluşturamadığı ifade edilmektedir (5, 103). Gerçekten de, tıbbi literatüre baktığımızda, penil-
vajinal ilişkinin, psikolojik açıdan en sağlıklı cinsel ilişki şekli olduğunu; buna karşın anal
ilişkinin ruh sağlığını olumsuz yönde etkileyebildiğini görmekteyiz (5). Örneğin, anal ilişki ile
olgunlaşmamış psikolojik defans mekanizmalarının kullanımı (104, 105), blumia (106) ve
posttravmatik stres bozukluğu (107) gibi birtakım psikolojik sorunların bağlantılı olabileceği
rapor edilmektedir.

    Anal Intercourse
The Ruling on Anal Intercourse in Islam

The meaning of verses 222 and 223 of Surah Al-Baqarah is as follows:
"They ask you about menstruation. Say: 'It is a state of discomfort; therefore, keep away from women during menstruation and do not approach them until they are cleansed. When they have cleansed themselves, then come to them from where Allah has ordained for you. Indeed, Allah loves those who are constantly repentant and loves those who purify themselves.' Your wives are a tilth for you; so come to your tilth as you wish. Send forth good deeds for yourselves. Fear Allah and know that you will surely meet Him. Give glad tidings to the believers!"

These verses provide an answer to the Muslim men who asked the Prophet about menstruation and instruct them to abstain from sexual intercourse during the menstrual period. At the same time, it has been stated that anal intercourse (liwāt) is also prohibited by these verses. This is because the phrase "where Allah has ordained for you" mentioned in the verse is understood to mean the vaginal route (birth canal). Indeed, the next verse states: "Your wives are a tilth for you; so come to your tilth as you wish." The tilth of humankind is the woman's womb; the path to this tilth is the vagina. Furthermore, our Lord created the vagina as a station where semen is discharged and proceeds towards the uterus, and the rectum and anal canal as a passage through which feces passes just before being expelled. All the anatomical, histological, and biochemical characteristics possessed by these organs are precisely tailored to their respective functions. Therefore, it is appropriate to understand the route that Allah has commanded to be used for sexual intercourse as the vaginal route, which He created for this purpose. Hence, the expression in the verse, "come to your tilth as you wish," does not permit certain non-standard sexual practices such as anal intercourse, but rather allows for vaginal intercourse in different positions.

When we examine the narrations regarding the reasons for the revelation of the above verses, we can conclude that these verses were revealed primarily to correct two erroneous practices:
During the pre-Islamic era (Jahiliyyah), anal intercourse was quite common among some Arab tribes in Mecca. Individuals belonging to these tribes married women from Medina after the Hijra (migration) and wished to have anal intercourse with them. However, this situation disturbed some Medinan women, and the matter eventually reached the Prophet (1). Thus, through the above verses, the command to "come to them from where Allah has ordained for you" served as a warning to these companions who wished to engage in anal intercourse with their wives. The second error corrected by the verse was the superstitious belief of the Jews of Medina that "if a woman's vagina is approached from behind, the child born will be cross-eyed" (2). Indeed, when we look at the sacred texts of the Jews, we see that the face-to-face missionary position, where the man is on top and the woman is below, was recommended as the appropriate sexual intercourse position, and other positions (standing, sitting, woman on top, etc.) were condemned on the grounds that they were harmful (Talmud, Gittin, 70a). So, in the Quran, with the command "come to your tilth as you wish," these superstitious beliefs of the Jews were corrected, and different sexual intercourse positions were permitted, provided it is via the vaginal route.

In the Hadith literature, there are many different narrations transmitted from the Prophet stating that anal intercourse is forbidden (3).

Anal Intercourse from a Medical Perspective

According to research conducted, the prevalence of anal intercourse is increasing worldwide. Pornographic publications that portray anal intercourse as a natural part of sex are thought to play a significant role in this increase. However, epidemiological studies show that for a significant majority of women who engage in anal intercourse, anal sex is not experienced normally and without problems, and complaints of pain frequently occur (4). Numerous scientific studies conducted to date have demonstrated that sexual intercourse via the penis and vagina (penile-vaginal intercourse) is the healthiest form of sexual activity from a psychological and physiological perspective; the link between other sexual activities and health is either weaker or non-existent, or they adversely affect health, as in the cases of masturbation and anal sex (5). In this article, based on information obtained from an extensive literature review, I will attempt to summarize the potential drawbacks of anal intercourse under several headings.

Figure 1. Some medical risks associated with anal intercourse.

    1) The Thinness of the Rectal Mucosa
The rectal mucosa is considerably thinner than that of the vagina. While the vagina is lined with stratified squamous epithelium, the rectal mucosa is lined with simple columnar epithelium (6, 7). In one study, the average thickness of the vaginal epithelium was found to be 215.5 μm, whereas the average thickness of the rectal epithelium was found to be 24.6 μm (8). Due to this histological feature, the rectal mucosa is more susceptible to damage during sexual intercourse (9-12). Mucosal damage creates two significant medical risks: infection and cancer. The fact that the rectal mucosa is only one cell layer thick facilitates the passage and spread of potential infectious agents by breaching the mucosal barrier traumatized during anal intercourse, allowing them to reach deeper layers (13-18). Moreover, frequent anal intercourse over a long period, leading to repeated traumatization of the mucosa, can trigger the development of anal cancer (19). Indeed, studies have shown that long-term irritation of the anal mucosa and repetitive epithelial regeneration are associated with the risk of anal cancer (20).

    2) Lack of Lubrication
Blood flow in the vagina is at relatively low levels at rest. With sexual stimulation, vaginal blood flow gradually increases, and this, through a process called "transudation," causes a lubricating fluid to appear on the surface of the vagina. The resulting vaginal lubrication facilitates penile entry and sexual movements, enabling comfortable intercourse. Additionally, it is thought that Bartholin's glands, stimulated by parasympathetic signals, contribute somewhat to vaginal lubrication during intercourse by secreting mucus. In conclusion, the vagina can lubricate spontaneously during sexual intercourse (21, 22). However, the rectum does not have such a feature, and because the anorectal membranes cannot provide sufficient lubrication during sexual intercourse, the tendency for mucosal damage is much greater in anal intercourse compared to vaginal intercourse. Mucosal damage, as stated above, increases the risk of sexually transmitted infections and anal cancer.

    3) Suppression of the Immune System
Human semen can lead to the suppression of the immune system in two different ways, independent of infectious agents: One is the direct suppression of immunity via the immunosuppressive molecules it contains. These substances found in semen prevent the female body from mounting an immunological response against sperm antigens, thereby increasing the chance of sperm fertilizing the egg (23-25). A second pathway is the immune response targeting antigens in the semen (allogeneic immunization) turning against the body's own defense system through cross-reactivity due to structural similarity (lymphocytotoxic autoimmunity) (26-29). Although it may seem paradoxical, scientific studies have shown that semen can cause immunosuppression through both pathways. When compared to vaginal intercourse, immunosuppression due to semen exposure is much more pronounced in anal intercourse (5, 30). The most probable reason for this is that the components of semen can more easily reach vascular structures and immune cells because the rectal epithelium is very thin (25, 31, 32).

Nevertheless, there are some publications with different findings and views. For example, Handzel et al. (33) found that T lymphocyte values were not affected in individuals who engage in anal intercourse. Chacho et al. (34) could not find a statistically significant relationship between anal intercourse and the development of sperm antibodies. Wiley et al. (35) found that in HIV-infected individuals, rectal exposure to semen via anal intercourse increased CD4 cell loss and worsened the disease course; however, they suggested this could be due to other infectious agents transmitted through anal intercourse.

In conclusion, suppression of the immune system may be one of the factors responsible for the link between anal intercourse, sexually transmitted diseases, and anal cancer. However, further research is needed on this subject.

    4) Sexually Transmitted Infections
Due to the rectal mucosa's greater susceptibility to traumatic damage and possibly the immunosuppressive effects of rectal exposure to semen, anal intercourse carries a serious risk for sexually transmitted infections, especially HIV infection (36-45). Laboratory studies have shown that vaginal tissue is significantly more resistant to HIV infection than rectal tissue (46-49). So much so that the risk of HIV transmission during anal intercourse is almost 20 times higher than during vaginal intercourse (50-53).

    5) Anal Cancer
There are numerous scientific studies showing that anal intercourse increases the risk of anal cancer (19, 37, 54-66). Although the primary factor implicated in this strong link between anal intercourse and anal cancer is HPV infection, some scientific studies have shown that anal intercourse can trigger cancer development independently of HPV infection (19, 67). This situation may be linked to two important problems caused by anal intercourse: suppression of the immune system and chronic mucosal trauma. Namely, in situations where the immune system is suppressed, such as in organ transplants, the risk of anal cancer increases significantly (62, 63, 68), and scientific studies have shown that anal intercourse can have a pronounced immunosuppressive effect on the organism (69-76). Furthermore, due to the thinness of the rectal epithelium and insufficient lubrication, the likelihood of mucosal damage during anal intercourse is quite high, and repetitive epithelial regeneration is known to be associated with anal cancer risk (20).

    6) Infertility
The vagina is lined with stratified squamous epithelium. This thick layer acts as a barrier preventing the contents of semen from reaching vascular structures, contributing to preventing the development of an immune response, especially against sperm cells. The rectal epithelium, however, is only a single cell layer thick, and therefore, anal intercourse theoretically carries a higher risk for the development of antibodies against sperm. It is stated that these sperm antibodies, formed as a result of rectal exposure to semen, could lead to infertility (25, 77). However, despite these theoretical assumptions and even some experimental (78) and observational (79) findings, there are also studies showing no significant link between anal intercourse and the development of sperm antibodies (34, 80). Therefore, we can say that the existing studies are insufficient to reach a definitive conclusion.

    7) Rectal Feces
Theoretically, feces are not present/stored in the rectum. The rectum is only a passageway for feces. The entry of feces from the sigmoid colon into the rectum via mass movement triggers the sensation of defecation. In this case, defecation can be initiated or, if conditions are not suitable, delayed. When defecation is postponed, the feces that have entered the rectum return to the sigmoid colon; in the case of defecation, it is expelled through the anus. However, during this process, a small amount of fecal particles may remain in the rectum. Indeed, physicians can perform stool examination via digital rectal examination. That is, encountering fecal residue within a finger's distance past the anus is a common occurrence. Therefore, during anal intercourse, the glans penis can become contaminated with fecal particles and be contaminated by various microorganisms present in the feces.

Microorganisms transmitted to the penis through anal intercourse can cause urinary tract infections in men (81-85); they can also reach the female genitourinary system through subsequent penile-vaginal intercourse (82, 86-89). Even the history of anal intercourse in the mother emerges as the most important risk factor determined for the colonization of E.coli bacteria in newborn babies (90).

Figure 2. Risks posed by anal intercourse related to rectal feces.

    8) Anal Sphincter Damage
The muscles that surround the openings or passages in the body and enable these structures to close by contracting are called "sphincters." The "anal sphincter" muscles surrounding the anus keep the anus closed to make controlled defecation possible. Damage to these muscles for any reason can lead to the loss of bowel control and the involuntary passage of stool or gas (fecal incontinence).

During defecation, on one hand, the muscles in the wall of the large intestine contract to push the stool forward, and on the other hand, the internal sphincter muscles reflexively relax to open the orifice for the passage of stool (91). However, during anal intercourse, this reflex relaxation mechanism will not function properly (92). On the contrary, there are studies showing that during sexual arousal, the anal sphincters contract reflexively, probably to prevent the passage of stool or gas during sexual intercourse (93-95). The contracted sphincter tissue, which has not been able to relax sufficiently, can be traumatized to varying degrees due to the forceful sexual movements occurring during anal intercourse (96-98).

Miles et al. (97), in a study conducted on homosexual men, found that complaints of fecal incontinence were significantly more common in individuals who engage in anal intercourse compared to normal individuals of the same age group; they determined that the anal resting pressure profile changed and the maximum anal resting pressure decreased, concluding that anal sex is associated with decreased resting pressure in the anal canal and an increased risk of anal incontinence. This study found a negative correlation between the total number of partners with whom anal intercourse was experienced and the maximum anal resting pressure. This indicates that the decrease in resting pressure develops cumulatively due to anal intercourse. Additionally, maximum resting pressure and maximum squeeze pressure were found to be lower in individuals with incontinence. This finding suggests that incontinence complaints may be linked to these pressure reductions. Miles et al. proposed that the traumatization of the internal anal sphincter due to anal intercourse might be responsible for the decrease in anal resting pressure. Indeed, it is known that even if the external anal sphincter is somehow paralyzed, the resting pressure is not greatly affected by this, and therefore a significant portion of the anal canal resting pressure is attributable to the internal anal sphincter (99). Chun et al. (100), although they found anal canal resting pressure to be significantly lower in homosexual men who engage in anal intercourse, did not find any evidence of structural damage in the internal or external anal sphincter. However, they stated that, although they found the difference not statistically significant, men engaging in anal intercourse tended to have thinner anal sphincters. Another noteworthy point is that the individuals Chun et al. included in their study did not have complaints of fecal incontinence. This suggests that the difference observed between the results of the studies conducted by Miles et al. and Chun et al. may be related to the selected patient group.

It appears that during anal intercourse, the failure of especially the internal anal sphincter to relax sufficiently results in a traumatic effect of varying degrees depending on the nature and quantity of the forceful movements. While this effect may not cause permanent structural damage to the sphincter, resulting in no complaints (100), it can also lead to serious structural problems progressing to fecal incontinence (97). Fecal incontinence can be general or may occur only under certain conditions. For instance, it has been reported that some patients with normal external anal sphincter EMGs but lower activity detected in internal anal sphincter EMGs, who could normally control stool and gas, experienced gas leakage during sexual intercourse (101). This indicates that a more detailed questioning should be conducted in risk groups.

Figure 3. Decrease in anal resting pressure due to anal intercourse (97).

    9) Biochemical Properties of Semen
Semen is a highly complex chemical mixture. Scientific research conducted especially in the last 30 years has greatly expanded our knowledge about the components of semen. However, we still possess very limited knowledge regarding the local and systemic effects of the vagina's or rectum's exposure to these molecules. Some important biochemical molecules found in semen include: LH, FSH, prolactin, vasopressin, oxytocin, TRH, serotonin, melatonin, catecholamines, endorphins, testosterone and its derivatives, estrogens, cortisol, prostaglandins, calcitonin, HCG, placental proteins, and relaxin (102). It is stated that some hormones like testosterone, LH, and oxytocin, as well as certain other chemicals like prostaglandins found in semen deposited in the vagina, provide psychological benefits for women; however, these substances cannot produce the same effect through the digestive tract (e.g., anal intercourse) (5, 103). Indeed, examining the medical literature, we see that penile-vaginal intercourse is the psychologically healthiest form of sexual activity, whereas anal intercourse can negatively affect mental health (5). For example, it is reported that anal intercourse may be linked to certain psychological problems such as the use of immature psychological defense mechanisms (104, 105), bulimia (106), and post-traumatic stress disorder (107). Future research on the biochemical molecules constituting semen and their local and systemic effects on the female body via the vaginal or rectal route may greatly advance our current knowledge regarding the benefits of vaginal intercourse and the risks posed by anal intercourse.

    10) The Transit Pathway Between the Vagina and the Brain: The Vagus Nerve
An important element that distinguishes sexual stimulation arising from the vagina from that of other sexual activities is the vagus nerve. Sensation from the clitoris is transmitted via the pudendal nerve. The pudendal nerve also collects sensation from the skin of the genital area and the anal canal. The pelvic and hypogastric nerves receive sensory stimuli from a large part of the female reproductive system, including the uterus, cervix, and vagina. These sensory signals from the pudendal, pelvic, and hypogastric nerves are primarily transmitted to the spinal cord. However, experimental studies and clinical observations have revealed that the vagus nerve, which is not related to the spinal cord, also carries sensory information from the reproductive organs, and it has been shown that vaginal stimulation can lead to orgasm even in women with complete spinal cord injury at the thoracic level (108-110).

Figure 4. Transmission of vaginal sensation directly to the brain via the vagus nerve.

This direct connection between the vagina and the brain via the vagus nerve causes vaginal intercourse to produce certain biochemical effects in the brain tissue that are different from other forms of sexual intercourse. This situation also triggers various physiological changes affecting the entire body.

The ability of the intervals between heartbeats in healthy individuals not to remain constant under different circumstances and to show periodic changes is called "heart rate variability (HRV)." HRV, which provides information about the sympathetic-parasympathetic balance in the body and is accepted as an indicator of both physiological and psychological health, is known to decrease in various diseases. Studies have shown that more frequent penile-vaginal intercourse is associated with better HRV; this association could not be detected for other forms of sexual activity (111, 112). A high blood pressure response to stressful situations (blood pressure reactivity) is also considered a risk factor for many diseases. In a study conducted by Brody, the frequency of penile-vaginal intercourse was found to be associated with lower blood pressure reactivity (113). The connection between penile-vaginal intercourse and greater HRV and lower blood pressure reactivity is thought to be potentially related to the activity of the vagus nerve, which connects the vagina directly to the brain (5).

    Conclusion
The anatomical and histological features of the rectum and anal canal are created to fulfill the function of defecation. The use of these structures for sexual intercourse carries significant medical risks. Therefore, removing anal intercourse from the sexual repertoire is a necessity not only from an Islamic perspective but also from a health perspective.

  • I Rapporti Anali

Viene affermato che questi anticorpi anti-sperma, formati a seguito dell'esposizione rettale allo sperma, potrebbero portare all'infertilità (25, 77).

Alcune importanti molecole biochimiche presenti nello sperma includono: LH, FSH, prolattina, vasopressina, ossitocina, TRH, serotonina, melatonina, catecolamine, endorfine, testosterone e suoi derivati, estrogeni, cortisolo, prostaglandine, calcitonina, HCG, proteine placentari e relaxina (102).
